Registering for Online Services

As from 1st April 2015 GP surgeries are contractually required to give patients online access to parts of their medical record (allergies, medications, reactions), in addition to the existing access to appointments and repeat prescription requests.  As a Practice, we have also decided that patients can have access to immunisation information and test results.

If you would like to request access to any of the above services, your identity will need to be verified by a Practice member of staff before the secure access details are given to you.  Please follow the guidance below.

  1. Please read the Patient Information Leaflet available here making sure you understand the potential risks of accessing your medical record on a computer, laptop, tablet or mobile phone.  A copy of the leaflet is also available from the Practice.
  2. Print off and complete an application form available here and bring it with you to the Surgery.  A copy of the form is also available from the Practice.
  3. As the receptionist will need to verify your identity before they can issue you with your personal log-in details, you will need to bring photo identification with you, for example a driving licence, passport or bus pass.
  4. The receptionist will print off an information sheet containing your secure access details.
  5. You will then need to register for the service by clicking here and follow the instructions provided on the sheet given to you by the receptionist.
  6. Once you have registered you can log-on to the Patient Access website by clicking here or clicking on the Online Services icon that is present on our homepage.

Please note the following:

  • The Practice has made the decision that, at present, only patients 16 years or over can request access to their medical records.  Parents/guardians will not be able to request access for their children until the Partners have obtained clarification on this issue and developed a Practice protocol.
  • Test Results - the abnormal/normal comment against a result is generated by the laboratory and not by the doctor and this will often reflect minor/ insignificant results that do not require intervention.  Your doctor will have checked the results with the knowledge of your full medical history and will make the decision whether further action is required.
  • Please note that it may take up to a week for all the online services you have requested to become available.
